Chicago Midwest Home Health Inc. has a comprehensive healthcare program. It is designed to provide a continuum of care tailored to each patient's needs. Patients who are home bound due to illness or disability are provided with intermittent  visits by skilled health professionals. Our dedicated staff cares for and helps patients transition at home surgery, skilled nursing facility stay or hospitalization and nursing home placements when patient care can be safely provided at home.

Home bound defined as the normal inability to leave home and leaving home would. require a considerable and taxing effort. Patients must require assistance of another person or medical device. Medical treatment away from home may be received but in general, time away from home should not longer than 1-2 hours or occur more often than 1-2 times per week.

Any Client requiring TDD or TT Devices as a method of communication, Our Home Health will contact the Illinois Telecommunication Access Corporation (ITAC) to arrange for their services.

Eligibility for Medicare coverage for home health care requires that a patient be under the care of a physician who will direct the plan of care and the patient be essentially home bound.
